Firstly this is a MUST BUY 5* book with class leading content. It's authorative, well researched, balanced, technically sound, easy to read. An asset to expert and lay person BUT.. why does it only get 3*s?
It is a text and reference book that is rendered almost unusable by its very thin and error riddled index and its fondness for using boxes that are not listed in the Contents. Finding one of the many valuable things that you have read is like looking for a needle in a haystack. In case you are looking for the "inverse care law" it is on page 212 not 214 as stated in the index. The index of just 5 pages for a 519 page book, is a token effort. It does not include names of authors and sources. Going to the Bibliography doesn't help. It is 25 pages long and listed alphabetically (Harvard style) which is hopeless for tracing back to where the work is used in the text. You can't track back to the chapter let alone the page where the material is quoted.
The easiest solution to this nightmare is for this EXCELLENT book (it really is. I have bought 2 copies !) to be published on Kindle so that it is possible to electronically search for data.
