In short a fantastic book. This book must be a wake-up call for all accident investigation branches & Safety Depts. around the world, to start analyzing incidents & accidents with an even larger focus on human factors.

This will hopefully also begin a slow process of teaching lawmakers, lawyers, judges and the public, that finding someone to blame, for an incidents or accidentmight not be the right answer for society as a whole, in the future. (Ref. Sydney Dekker "Just Culture".)

I found a book review of this by a 330 training captain. I have to agree with him that these incidents and their analysis leave you wondering whether, given the circumstances, other pilots would have acted differently? A good read and chilling food for though about the limitations of some of our training resources.

Fantastic book, valuable for all aviation professionals. I can easily recommend it, as it should be present in every aviator's library.
