How does this work, do I have to have two SIM cards one for the watch and one for the phone

The watch runs app extensions for apps on your phone via Bluetooth, it does not then need a sim card. In some ways it is simply another screen for your phone, with a different layout, and obviously not full capabilities.

No, all the information on the watch comes direct to the phone via Bluetooth. If you do decide to buy, a good app for this phone is Augmented Smart Watch pro.

It works via bluetooth connection with your phone. You don't need two SIM cards.
