An update, 18 months later my daughter is like a different child. After an 18 week CBT therapy course with CAHMS, our daughter worries about very little, and if she does, she is able to contain it and not let the worries get out of control. I wanted to give desperate parents reassurance that things can get better. We still have bed time worries, but I must highly recommend Bed Tim Meditation for Kids CD's by Christanne Kerr. Good luck!
