The "green" PHTLS book has a handful chapters (100 pages plus) in addition to the normal "red" book. It has all that the red book has, word for word, so it is no problem to use it on a civilian course. But it seems that a little bit of extra work has gone into the military chapters. And a couple of these may bring some for the civilian user too. The military chapter on bomb blasts seemed to fill out the civilian Chapter 19 with regards to some extra quality.
The drawback is that the book is thicker and heavier. The sheer size can be a tad too much.
Very well written, this is my core text book and find myself referring to it time and time again. Useful pocket memo's and backed up with a CD it puts the information over in a very digestible manner. Whilst it's very American orientated most of the terminology and facts transferrable. I wouldn't be without this now.
