I have worked through this book. Since rigging was a new area for me, it was great to find a handy reference book. Really frustrating to go through the book using your own mesh and to rigged following the book, step by step and then for things not to work,because of course you think you have made the mistake as you are a Newbie at this. No, the Editor did not do their job. So if you plan to use this book make sure you cross reference to the examples on the accompanying DVD. Lets hope for everyone sanity for the next edition they actually check out what is written.
Looking forward to Mancandy DVD since this book did get me through the rigging basics.

It's great to hear that the book got you through the rigging basics. Mission accomplished! Of course, like any book, there are a few errors, which are listed on the errata page of the book's Sybex website. But as you correctly pointed out, the DVD has all you need to get through these rough patches, just in case (which of course was precisely the reason why the files were included on the DVD in the first place). So I'm very glad to hear you found the DVD to be a helpful addition to the book!

Learning Blender is a challenge. I'm glad the book was able to help you!
