John Nance's book tells the story of a Fictional US hospital that undergoes a major transformation to become an institute totally focused on safety and quality care. It inspires and challenges. It is written in a easy to read narrative with a backstory of a physician who was a previous CEO who stood down following a fatal error in his hospital who visits to see how safety could be achieved.

As an understating Brit I found some of the story a bit syrupy (hence only 4 stars), but it has important take home messages including: effects of fatigue/hunger/ dehydration on human performance, empowering the whole team, how focused on finance boards can be.

I will recommend this book to two groups colleagues (especially doctors in training, and nurses moving to managerial roles) who want to know about safety and transformation, and colleagues (especially senior doctors and managers) who are becoming focused on financial and performance targets, not quality of care.

Oh my word, what an amazing book. This should be compulsory reading for healthcare professionals. I work in an urban ER in Scotland and the themes described in the book resonate enormously. Although we have very different healthcare systems, we have the same problems and I hope that this book can be an inspiration for us to radically change our systems and ultimately improve patient safety. Thank you so much.

Essential reading for all NHS staff.
Never events should never happen if checklists are followed diligently. Attention to detail is always essential.
