What is glucose monitoring?Glucose monitoring is a way of testing the concentration of glucose in blood samples. Blood glucose monitors allow you to keep track of your levels and take action when necessary.Glucose monitor kits usually consist of a monitor and a lancing device, which looks a bit like a pen. There are many different types available some may be easier to use and some give more detailed information. You should try and decide which is the best monitor for you.What can make my blood glucose levels rise and fall?During the day your blood glucose levels will vary and there are many factors that can cause these changes, these are as follows:
Why do I need a Blood Glucose Monitor- Eating - especially if you eat sweet or starch based foods or miss meals.
- Stress - if you are feeling stressed your body's natural defences will produce more glucose.
- Alcohol - Can cause your levels to drop because it shuts down your body's natural mechanism for raising blood glucose.
- Exercise - Usually causes blood glucose levels to drop unless you do not have enough insulin then levels can rise.
- Smoking - Can make your body less sensitive to insulin.
- Illness - your body will produce more glucose to help fight infection, so even if you are not eating your glucose levels can still be raised.
- 1. Ensure all strips are in date and that your machine if previously used is kept clean.
- 2. Wash your hands and rinse well with warm water, the blood will flow better with warm hands. Also the hands need to be clean to get an accurate result and to remove any substances that may effect the result. If you are going out and are not sure if there will be places available to wash your hands then you should carry a wet flannel in a plastic bag to clean your fingers with. Avoid the use of wipes as these could effect the result.
- 3. Choose which finger you are going to use to take the blood sample. Massage this finger well to improve the circulation.
- 4. Pricking the fleshy part of the skin can hurt, prick the side of the finger away from the thumb.
- 5. Once you have punctured the skin, squeeze your finger gently to obtain a drop of blood.
- 6. Apply the blood to the testing strip.

MQ 2012-1 Comparison of glucose meters with heparin whole blood

Introduction
Glucose meters for self-testing patients are provided for the analysis of fresh capillary blood.
The accuracy of the devices can’t be compared because collaborative studies use plasma based controls. This experiment with available blood glucose meters in Switzerland should provide you with information’s of the comparability of glucose measurements with these
devices.
Procedure
Devices and test strips have been provided by the manufacturers. All devices were tested
with the control solutions by the manufacturer and were fine. At the first test we have stored
venous blood for 3 hours at room temperature. In the second sample we analysed postprandial venous blood immediately after collection. All measurements were carried out within 30 minutes.
Additional Measurements
Oxygen content of both samples was monitored with a Axonlab of ISTAT. It remained in both
samples over the entire period of less than 5 kPa PO2. After centrifugation the glucose was
determined using a Cobas 8000 (hexokinase method) and was 4.3 mmol / l at the first and
6.5 mmol / l at the second sample.
Discussion
Precision
To remain in the QUALAB tolerance of 10%, the CV% should not be greater than 5%.
It is optimal if it is less than 3.3%.
First sample was let 3 hours at room temperature. The sample was clearer viscous than sample 2 which was probably a reason for the poor precision for all devices.
The second sample was fresh and had a higher glucose concentration. At 12 units he CV% was less than 3.3%. The 5% mark was exceeded by three units.
Accuracy
To assess the accuracy of the devices we use two clues. On the one we calculate the average of all measured values of all devices, on the other we measure at the beginning, middle and end of the experiment the glucose on a Roche Cobas 8000 at the University Hospital in Zurich.
The average of the three measurements is used in the chart as "target value". In addition we use an ISTAT allows us to glucose and blood gases directly from the Whole blood sample to measure.
The large variation in sample 1 meant that some values are more than 10% away from the target value. The average value of all equipment was exactly the same as the value of the Cobas 8000.
Also in Sample 2 average was equal to the value of the Cobas 8000. Because of the improved precision all values were within the + / - 10% range around the target value.

Summury
Basically, the 20 meters were all tested 10 times against a laboratory verified sample of 6.5mmol/l. The results above show that only 3 meters achieved a more accurate result (shown by the "bias" column) over the 10 tests but of these 3, only 1 meter achieved a more consistently accurate result (shown by the variance figures in the "VK%" column). In other words, of the 3 meters that had a more accurate result over the 10 tests, 2 of them had much greater variation of results (i.e. less consistency).
These results also show that whilst the SD Codefree Meter will consistently give you an accurate result, some better known brands such as the Freestyle Lite and the Bayer Contour will consistently give you readings which are much lower than your true blood glucose level.
