Urine burns on a cat

I have a 4 year old cat who was run over as a kitten and had 75% of his urethra removed, so now wees through a hole in his tummy.
He leaks a little urine from there but otherwise has made a remarkable recovery. Unfortunately these leaks irritate him and he licks the area a lot. He often develops nasty ulcers there, requiring anti biotics. I clean the area once a day with hibiscrub, which helps, he has a grain-free diet and daily homeopathic remedy suggested by the vet to reduce the acidity of his urine. However, I noticed that when recovering from an operation and wearing a head collar his condition improved dramatically, so his vet and I think that his licking irritates the area. Does anyone have any suggestions about how to stop him doing this please?
