In the style of Barbara Erskine (but not her) the story is about a rather unpleasant young woman in the present day who has a serious accident/illness
While in a coma she is transported to a time in the long ago where she is much nicer, she falls in love.
Eventually she decides she wants to stay in the past, whereupon her present day body dies, leaving her to enjoy the 'life' she has chosen.
Does this sound familiar to anyone? I would love to read it again if I could only track it down.
