With no explanation, a sadistic madman (played by Shigeo Osako) degrades, tortures and mutilates them. For those who like their extreme asian horror based around sadism, sexual violence and torture, will love this.

Two lovers get kidnapped by a sadistic surgeon and are kept in his purpose built house, only to be tortured and psychologically manipulated. Pretty simple plot, the surgeon pits the two against each other over what seems to be an extreme pain threshold. The main theme is actually not of torture and horror but of the two lovers relationship and how far they are willing to go.

The reason why this has been banned by the BBFC is basically down to the nature of the torture, its generally based on sexual sadism and we all know how the BBFC feels about that.
This of course is ludicrous, this film will only appeal to those who love extreme asian horror, and follow the cult, more underground movies, as it isnt meant to be a commercially driven film. Its also not nearly as disturbing as others like GUINEA PIGs and THE BUTCHER,(and many many more!). In fact the most extreme film that the BBFC so far has 'allowed' is supposedly MEAT GRINDER, which actually prompted me to write this review. A comment in the Times argued that these films should all be cut or banned - again missing the point that these arent commercially viable films.
I actually assumed that the film had to be uncut just because there is a constant onslaught of sexual sadism, so where would you begin?

We also have to remember that Japan especially, started releasing these sort of films in the 70s so its nothing new.
As some1 previously mentioned this is tame compared to alot of other cult extreme films. MURDER-SET-PIECES for instance was initially cut to shreds, once restored it became a great nasty horror film.

If you're anything like me, the first question you'll want answering will be: Is this really the uncut version of the banned Jap torture/porn flick? Yes, with none of its goriness spared. Next, Should it have been banned or are the BBFC overreacting? Well, in my humble opinion, The Butcher (Korean release) and Murder Set Pieces were far more disturbing than this one; the sadistic torturer is not in the same league as Kakihara and the director, Koji Shiraishi, doesn't bring the pain as explicitly as Miike's Ichi The Killer. Grotesque does lack the moral compass required in the eyes of the BBFC and that is the only reason for the ban. Realistically, does anybody watch this kind of flick to be given a moral lesson in the virtues of life? No! Will it encourage any normal person to commit random acts of unpleasantness against their fellow man? No, unless your intellectual quotient is the equivalent of a bedroom slipper and you have the slight disposition of being remotely unhinged in the first place. So, if you've ticked all the right boxes thus far and passed the compos mentus test of acceptance here's a brief insight into one of the cruellest splatter flicks on the market... remember, it is just a movie!!

A young couple, on their first date, get kidnapped by a sadistic surgeon who gets sexual gratification from watching his victims beg for their lives as he subjects them to acts of degradation and humiliation with the promise that the other one will live if they endure his sick, twisted game. Fingers are chopped off one victim and given to the other on a necklace as a gift, nipples are severed, love spuds get nailed in genital mutilation and limbs will be hacked off.

I totally disagree with all the rest of the reviews. this movie most certainly is the top horror and no other movie that exists even comes close to how violent cruel sad sick gruesome hard to stomach this is. murder set pieces is not at all disturbing. I have never seen the guinea pigs yet but I really would doubt them being any where near as sick as this movie.
