This is an interesting book in the field of Root Cause Analysis. In some respects it's clearly been designed for use by beginners - and the chapter on identifying the problem is one of the best I've read. For that alone, it's worth the price.
But for me, this book falls down when it moves into the analysis of an incident. Whilst cause and effect charting is a very effective technique, it's not one I would advocate for complete beginners, or at least, not without considerable more depth and support than that given here.
