In the past decade, important progress has been made in understanding the spectrum of obsessive-compulsive disorders; however, most advances in treatment have been directed at the compulsive element of the behaviour. It is estimated that for as many as one in three patients presenting with obsessive-compulsive disorder, the primary problem is the obsessions. In a previous review of his book, Rachman was unfairly criticised for not addressing issues such as fandom, or football fanaticism. However, this review failed on even a basic level to grasp the author's main purpose: to expand upon his previous work by proposing, for an audience of academics and clinical professionals, a new cognitive theory of obsessions (by which is meant the distressing and "unwanted thoughts, images or impulses" experienced continously by many sufferers of obsessive-compulsive disorders) and from this to outline a new treatment method for use and adaptation by clinical practitioners of cognitive behvioural therapy. As such, the book is a resounding sucess.

As a sufferer of deeply distressing obsessions throughout my adolescence and early adulthood, I found the book to be enlightening. By considering my obsessions as extreme manifestations of a normal psychological phenomenon - the intrusion of meaningless mental flotsam - and using this observation as the basis for behavioural and cognitive experiments, I have manged to reduce the significance of my obsessions, their impact upon my life, and furthermore have experienced a great reduction in their frequency.

Although this book was recommended to me by a therapist, it should be noted that it is not intended as a self-help manual for patients, and caution should therefore be exercised if it is used by someone suffering with obsessions who is not under the supervision of mental healthcare professionals. However, so clear and logically presented are Rachman's thoughts that academic and educated lay reader alike will find an insightful and stimulating approach to the treatment of obsessions, whose results are at best life-altering for patients. I wholeheartedly recommend this book.
