oh dear I cannot be much help as I do not own a pair of trainers! I would think though, because the orthotics are quite deep you would probably wear them instead of the insole. When they are inside your shoe your feet feel "higher" than normal. I take a size 5 shoe, and the chiropodist told me I need the small size Orthaheel...and it is fine. I would think this size would be fine for a size 4 shoe as well, because being just the heel part of the shoe the size is not too critical.
