This powerful book written by Dr Derek Doyle, one of the charismatic pioneers of palliative care, will make you laugh and it will make you cry. The text describes a wealth of anecdotes from Derek's day to day life as the medical director in St Columba's Hospice in Edinburgh. He is an accomplished medical author who has the rare ability to write for many different readers. One of the greatest pleasures of reading his work is the rich sense of humanity which leaps from the page - this is indeed an enviable skill for a health professional. In addition I believe the book should be treasured for its place in the social history of the hospice movement. It describes the holistic medicine practiced by consultant physicians in hospice care the UK, which may be at risk as junior doctors are increasingly introduced. This is a book to read and enjoy and then tuck away for those days when the burden of caring becomes too great. This is a book one could never tire of, I believe it is destined to be a classic.

If you have ever cared for a dying friend, relative or patient you should read this book. Derek Doyle, an international expert in caring for the dying, shares his extensive experience as a hospice doctor. Every page is a gift. The reader is taken on a remarkable journey as Derek describes how he accompanied his patients and families as they prepared for death. Don't expect to feel depressed because this gem of a book exudes hope and love Of course there are sad stories of families torn apart by advancing illness who are reconciled when time is short and regret abounds. Other families, despite valiant efforts of the professional carers, never manage to resolve their differences.

As you read on you will inevitably consider your own mortality as you learn how others faced death. You can expect to feel profoundly moved, but also comforted as you understand how others have come to terms with dying.

As well as Derek's considerable medical experience the reader comes to know his greatest strength - his warmth and humility. If you have met Derek you will notice that he calls you "friend". He means it!

Anyone who is curious about their forthcoming journey will benefit from reading this book. It should be at the top of the reading list of every health care professional in every country. All who read it will feel enormously enriched.

A little gem 26 April 2000

This book is a little gem. Dr. Doyle shares with us the most powerful and moving experiences from his life as a palliative care physician. Rarely was I so moved by a book, laughing out loud several times and crying silently even more often. It is the patients who teach the physicians the most important lessons when, at the end of their lives, they realize what is truly important and are able to share it. What is needed here on the physicians' part is the difficult art of listening, and Dr. Doyle gives us some beautiful examples of how tremendously helpful this can be. This book was a real gift to me. I hope it will find many readers, not only among palliative care specialists. Why the intriguing title? Well, you'll have to read it to find out!
