This treatment manual details in a step-by-step and easy-to-follow manner the Sorensen Therapy for Instability in Mood (STIM), which is an important new psycho-educational and cognitive therapy for bipolar disorder (BPD, formerly referred to as manic depression). The key characteristics of the therapy are that it is brief, taking place over only four 60-minute sessions, and that it can be delivered by practitioners with little specialist training. The author, John Sorensen, has achieved impressive results with patients who have received the therapy, with significant improvements in their perceived control over mood, and significantly reduced hopelessness, which is known to be closely related to the likelihood of attempting suicide.
Historically, there has been a reluctance to offer psychotherapy to patients with bipolar disorder as they were perceived to be unable to benefit from talking treatments – the standard treatment is drug-based. However, STIM is a psychological therapy which has proven to be effective, popular with clients, inexpensive to implement and one that delivers rapid results. It is based on the collaborative development of a relapse-prevention handbook that also provides the client with an individualised bio-psycho-social formulation of their BPD-related experiences. Education about the disorder, finding a client’s particular triggers for depression and mania, tailor-made coping strategies, and the inclusion of the patient’s social network in attempts to stabilise mood are the key elements.

John Sorensen is a Clinical Psychologist at North Manchester General Hospital and has worked in mental health departments in Denmark and in the UK. Throughout his professional life Dr Sorensen has been committed to the development of treatments and approaches that empower service users and are based on the needs of the individual.

As a psychiatric social worker I have now used this book with two clients and both have got significant benefit from it. Sorensen's book outlines four workshops; one on the nature of bipolar disorder, one focusing on depression and supporting the reader to map his or her own symptoms and triggers, another doing the same thing for the manic side, and a fourth chapter which looks at managing the condition in the context of relationships and work situations. The book is practical, free of jargon, and supports the reader to note exactly how the illness works in his or her case. My clients focused on different aspects of the book as particularly helpful and I now plan to work with further clients and colleagues of mine are also working with it. My clients have made significant gains in insight and it will be interesting to see the long-term benefit over time.

So simple and short-term (4 sessions!!), yet so comprehensive. I have been looking for something along these lines for years and I really believe that this could be used across mental health services and even by GPs or practice nurses who need something quick, easy to implement and yet satisfying for clients who want to be responsible partners in maintaining their mood stability. Great to use!

This book is really a programme of action and change to attitudes, so your patients need to be ready to confront how they may have been 'mismanaging' their mood instability for years. If you are willing to do that work with them, the book is excellent, if not it is a waste of time. I personally liked its practical, no nonsense approach and the fact that it does not just have one 'truth' but allows you to find what works for the individual person as you go along. Also the workbook gives a permanent record of what you develop and it contains strategies and handouts for family and friends, which I have found useful.
Highly recommended partly for its simple, easy to use, format that does not require extensive training or preparation.
