The Medela 27 mm Large PersonalFit Breastshield is for larger than average nipples--because one size does not fit all. Medela's wide range of PersonalFit breastshields allows mothers to choose the breastshield that is the right size for them, to ensure maximum comfort and efficiency when pumping. Designed for individual maximum comfort, personalised breastshield size for most comfortable pumping, maximum output, optimised breast emptying. Made from 100% Bisphenol-A free materials for the health of your baby.The right breastshield size and a proper fit avoid compression of any milk ducts during pumping. As a consequence the mother is given the best chance of protecting of her milk volume, optimising breast emptying, preventing nipple trauma and pumping with comfort. This will help a mother to maintain her milk production and support her lactation. Using a too small or too large breastshield may lead to impaired milk removal because milk ducts could get blocked or milk ejection inhibited. Over time, this can result in reduced daily milk output, which will decrease milk production and can lead to early weaning. Worth buying a breast shield that fits you personally was struggling with the 24mm shield, and got very little milk and was painful but very glad that got new shield. Only wish you could choose size when you buy the pump

I bought this because the standard one that came with my pump was, as I learned, too small for me and partly the reason it was so uncomfortable and unproductive to pump. I wish they would tell you these things in the instruction manual or something so you could have an idea of how to size yourself properly for optimal comfort and flow. Overall this works OK, it is very easy to clean and sterilise and I'd recommend it if the standard size is a bit too small for you. However, after I discovered their softfit breastshield, which is comfortable for nipples of a large range of sizes, I soon chucked this. It is SO much softer and more comfortable and I no longer get horrible sores around my nipples from the hard plastic.

It is very important to have the right size for the breastshield otherwise pomping hurts and the quantity is not so great.
The problems is how to know which size is the best for you. I think that if you have large nipples with large aureoles you should definitively go for large.

A must have if the standard doesn`t fit!
I purchased this after having issues with the standard Medela shield, as it was too small, an since using the bigger size, I have been able to pump more effectively!
