Book Description

15 Dec 1997 1840141050 978-1840141054 illustrated edition
This is a practical book aimed at those whose daily task it is to think about and manage or regulate the risks of hazardous technologies. The book is not targeted at any one domain, but attempts to identify general tools and principles that are applicable to all organizations facing dangers of one sort or another. This could include banks and building societies just as much as nuclear power plants, oil exploration and production, chemical process plants, and air, sea and rail transport. The emphasis is placed upon the principles and practicalities of defenses against accidents, and how to meet the challenges and minimize risk.

5.0 out of 5 stars First class 28 Jan 2008
Format:Paperback
This is an absolutely splendid book - full of insights and information, new ideas and worthwhile approaches applicable to all manner of industrial situations. I made it a student book at an MSc post-graduate university course because of its value and originality of its thinking. It is rare and special and more readable that his "Human Error".

5.0 out of 5 stars A must have for anyone working within the field 25 Oct 2002
Format:Paperback
This book is the perfect companion to J.Reasons Human Error.
There is some overlap between the books but they complement each other nicely.

The strenghts of this book is that it will give you insights, mental models, methods and principles for working with risks/Organizational Accidents/Human Error

I just wish that the chapter "Practical Guide to Error Management" and "Engineering a safety culture" would contain a bit more examples and information.

Over 10 years on while Ch 7 has dated but the rest remains essential reading. My personal best 'swiss cheese conference' was one in which 5 presenters referenced Reason's most famous model.
