Book binding faulty with ripped pages 5 Feb 2011

Great book with good content but I was concerned on finding several pages ripped and the binding seemingly coming apart. I don't want to go through the hassle of returning so I fixed as well as I can. The problems were located in chapter 8. Also, the author supplies an outdated email contact to request further data and a spreadsheet.
