This book is as important as any I have read in the last five years. It is a wake up call to each and every one of us whether we be in high office or a concerned individual. This work combines the best thinking in popular psychology, social history, economics, politics and rationality. Most of the major catastrophies in recent history have been significantly the consequence of faulty human thinking and behaviour. These are sobering and at times shocking lessons. For example there are tales of hair raising cowardace and greed and arrogance which subsequently cost many many lives. But there is great optimism in the book too, for it diagnoses these faulty cognitions and articulates factual practical and actionable solutions. Any human being who cares about the crises we presently face in the health service, in schools, in social behaviour, in foreign policy, should read this book and also send a copy to anyone they know in public office.

The easiest thing for a CEO, senior manager or policy maker to do is manage away inconvenient truth or fact... and to put difficult facts or advice into familiar and corporately convenient contexts. What Dr. Gerstein's very important book deals with is the fundemental need to understand the processes by which decisions are made, or not made! That organzations rarely ever learn from past errors or disasters. I suppose that means that history does repeat itself, over and over again, clearly to the risk of all. But this book is important in others ways, it assesses and highlights the complexity of the decision process in a comprehensive way that will make all readers think about their own circumstances and how perhaps they can avoid the traps of present and future errors of judgement.
A very highly recommended read which is timely and thought provoking.
