I listen to audio books on my iPhone when out hill-walking and also want to be able to use the phone. The ear-buds with mic you get with an iPhone work well, but I find that the earpieces can fall out when exercising or in windy weather and so bought this Belkin adapter that enables me to use more secure headphones without losing the ability to talk on the phone. I've been out testing the equipment in the hills and find that the device works fine. The sound quality was good and I could hear when someone phoned and they could hear me. The socket for plugging in your own headphones is near the plug that fits into the iPhone and the small, rectangular part at the other end of the cable houses the microphone, on/off answering button and the volume control. The buttons are very close together and someone with large fingers might find it a bit fiddly. Down the side of the microphone part is a narrow groove designed to slot in the cable of your headphones. I find it doesn't hold the headphone cable securely. You also get 4 cable clips which are useless. I've taken to securing the mic with a wire tie or elastic band.

Update October 2011: I've just found another benefit of this device. If you're out walking in colder weather and have your iPhone secreted away in a pocket and are listening through headphones connected via a cable tat is snaking up under clothing, then you can have the microphone/volume control cable of the Belkin device on the outside for easy access to on/off/volume control and to the microphone for easy answering of a call. This is a great advantage for, in winter, I have found that I have sometimes miss a call as I scrabble around trying to get at the microphone/control buttons on my Apple headphones with mic hidden behind a neck gaitor and zipped up waterproof jacket.

I bought this hands free kit to work with my iPhone 4S (Siri) and my Sennheiser IE8 headphones. I didn't want to give up my headphones for the terrible buds they provide on your regular handsfree kit, so this seemed like the ideal compromise.
Before I say what's good and not so good, overall, I like this a lot.

Pro's:
Excellent mic reception and clarity. I wear the mic UNDER my work shirt, near the top of my chest, and it picks me up loud and clear, speaking normally not shouting.
Slim, leightweight design that just feels like a normal part of a headphone cable.
Supports other functions with the iphone and Siri like, volume, skip tracks, start Siri etc.

Cons:
The provided 'clips' to clip the handsfree cable along your headphones cable are terrible. So your two cables just randomly flop about and tangle together unless you come up with a homemade solution.
The build quality of this device seems very low and fragile. It seems like it will fall apart any minute, it might last, I could be wrong.
Cable is very thin and flimsy, so where it joins to the headfone jack and to the mic, it feels like it will tear out of the moulding every time you touch it. Needs better mouldings into the ends.
The multi function buttons on the mic, the volume buttons work fine, but the middle button for the other iphone controls is a nightmare, very hit and miss. You have to try and find a balance, and hit the pad in the middle, a millimetre slightly off dead centre and you turn the volume up and down. I'd say my success rate is 50% at getting that button at best, but the volume buttons 100%. Bear in mind that you have to hold that middle button down for about 3 seconds to activate the iphone, so that can end up with music blasting out of your ears. and some functions need a triple click!! hahahahaha no chance.

But, as a handfree mic to connect to your existing headfones - this bit of kit is highly recommended. just be careful if you want to use the other features of it with a smartphone.

**EDIT**

The durability of this device is terrible. It gave up on me after about a week with virtually no abuse to it at all. Putting my phone in my trouser pocket was enough to ruin the headphone jack so now it changes tracks, calls people, changes volume, cuts me off - all by itself. Nice design but far too lightweight. Dropped my review from 4 stars to two. Buy another handfree, this one isn't upto the job.

Doesn't clip onto my earphones properly so I've had to tape it to them instead. Control is a bit fiddely. If you are buying this then note that your earphones plug into the bottom near the plug and not near the control. The control is supposed to attach to the top of your earphones and it comes with small clips that are supposed to attach it to the existing wire at various intervals.
If I do ever need to replace it I may try getting a different type.
Microphone functions reasonably though and it seems to be pretty well made.

Cumbersome
Item is a bit unwieldy as there is too much cable. The plug feels fragile and the channel to fit the existing cable into is too narrow for my Shure headphones.
